You should have your piano tuned as often as you feel necessary. As a rule of thumb, you should tune your piano at least every 6 months.
It might also be necessary to tune your piano more often, say every 3 months, if you play your piano daily for hours, or for a music school.
What happens if you do not tune your piano for a long time?
The piano will drop its tune too low below the concert pitch. You will need to tune your piano for a couple of sessions, or you might need to do a pitchraise, in order to help re-establish long-term stability. All these additional procedures will eventually incur higher cost.
If your piano pitch fall too much below the standard pitch, you will need to tune your piano more frequently and bring it back up to standard 440hz.
As a guide:
Pianos 10 to 20 cents (3 to 5hz) flat - should be tuned once again in 2 months; then every 6 months thereafter.
Pianos 20 to 25 cents (4 to 6hz) flat - should be tuned in 1 month; then every 6 months thereafter.
Pianos 25 cents (6hz) flat or more should be tuned again in 2-4 weeks; and then tuned again in 3 months; and then every 6 months thereafter.
